On 12 07 2016, the department of licenses and inspections issued a notice of refusal for application #738652.

Level iii structural alterations and repairs (no occupancy) to include infill construction of new third floor within existing building envelope as per approved plans. No partition construction on this permit. Separate permit required for interior fit-out prior to occupancy
