Installation of three small cell antennas, one ballast mounted and two flush mounted with related equipment mounted to side of building

Please extend permit an1500176. This permit expired on 6/19/16. This would be the first extension for the permit making it valid until 12/19/16

This is a revisions to already approved and issued permit: an1500176. The only change to this permit will that the antennas mounted along the edge of the building on the lower roof level, along the west elevation of the building -beta sector will have a screening of a faux chimney concealing the antennas from view, per anne brocketts request. The proposed chimney feature will match the color and brick design of the exiting building.
