Relocation of lot lines to create one (1) lot from four (4) lots (consolidation of 2324, 2334, 2336 & 2338 n. 6th st) - no changes to existing structure - will be used as religious assembly as previously approved

Amending permit# 455054 - legalize 200amp service installed by others, total rewire for general use light & receptacles, new fire alarm as per nec 2008 & nfpa 72 (church)

Install 3 wc, 3 lav, 1 sink

Remove existing gate and infill portion of front wall with reinforced cmu (including new double door opening with new lintel header); interior alterations throughout including partitions, altar, toilet rooms, seating areas, accessible elements and finsihes to create a place of worship; lawful occupancy sign and certificate of occupancy for complete change of occupancy in an exisitng one (1) story structure. Separate permits required for all mechanical, electrical and plumbing work. See affidavit of limited cooking for kitchen.
