The hotel manager on duty was previously instructed by the site fire chief on lock out provisions and the requirement to pull permits for the repairs. I walked to the area involved with the fire rep and we discussed with the maintenance personnel the lock out tag out procedure. I remained on site to verify the lock out tag out and provided instruction in the use of the scissor lock out device to the maintenance personnel. I remained on site and verified the lock out/tag out to completion. I have re affirmed to the personnel present to contact the elevator company for the permit and the repairs. Fire was contained in the elevator machine room and was a probable result of pumping equipment failure for the hydraulic system. The hotel manager on duty was previously instructed by the site fire chief on lock out provisions and the requirement to pull permits for the repairs. I walked to the area involved with the fire rep and we discussed with the maintenance personnel the lock out tag out procedure. I remained on site to verify the lock out tag out and provided instruction in the use of the scissor lock out device to the maintenance personnel. I remained on site and verified the lock out tag out to completion. I have re affirmed to the personnel present to contact the elevator company for the permit and the repairs. Fire was contained in the elevator machine room and was a probable result of pumping equipment failure for the hydraulic system. Any pin point cause would need to be determined by the elevator contractor as any speculation on my part would be pointless. Thank you for your time, don donald d.
